Please complete the following form to grant San Isabel Electric Association and its electric cooperative partners permission to use your likeness to support and promote the electric cooperative and various programs.
Please complete this Student Scholarship Check Request to notify San Isabel Electric where to send your scholarship funds. It is the responsibility of the student to ensure checks are delivered to her/his institution of choice in a timely manner.
Scholarship funds will be sent directly to the college or university of your choice. Please complete this form no later than June 30.
When a payment is made to your school, you and your parent/guardian will receive an email to inform you that a payment was made to your college. Payments are typically made in July. Schools typically split the scholarship equally between the fall and spring semesters.
Also, please complete the Photo Release Form at https://siea.submittable.com/submit/164732/photo-release and attach a Senior picture for SIEA to use In publications and social media.

Please check the box below to confirm that you have read all of the guidelines required to apply for a contribution. Not following instructions may result in disqualification. Please answer ALL of the questions.
Please complete this form if your organization has already received funding from San Isabel Electric and your project is complete. Donation reports must be completed within 30 days of the project's completion. Entities that have not submitted a donation report form within this time frame will not be considered for funding at a minimum, for the following year.
